Please ensure you have a copy of your child's Baptism certificate in PDF format before commencing this form -

it will need to be uploaded in order to be able to submit the form.

This includes children Baptised in Ss Peter and Paul Parish.

If you are unable to find the Baptism certificate please contact the parish of Baptism

for a copy before completing this form.

I agree to help my child prepare for their Sacraments by participating fully with them in the preparation program. This includes attending the Parent Information Night and the Commitment Mass, ensuring my child completes the lessons properly, and bringing my child to Mass to return their work.
